Why is the CP Register needed:
- to find out the causes of cerebral palsy (CP)
- to find ways to prevent and manage cerebral palsy
- to help in planning services for children and adults who have CP.
You can participate in a number of ways:
- Visit the CP Register website, become a member and submit the required details http://www.cpregister-aus.com.au
- Download the registration (
PDF) and consent forms (
PDF) , complete them and mail to:

- Contact us and register directly by phone 02 9479 7200.
- Give permission for your health practitioner or education professional to:
a) submit your details on the CP Register website, or
b) send your name and contact details to the Register.
We will contact you and obtain the required details.
